Floating support: young people at risk

Aims of the service

To provide a service to young people who still require a degree of support to enable them to live independently within the community in their own tenancy.

 

Who can apply for the service?

  • Anyone who is:
  • aged between 16 and 25
  • a single person with support needs
  • male or female
  • eligible for state benefits
  • present no significant risk of harming self or others
  • be able to make an informed choice.

 

How to access the service

Access to the service can be from:

  • a local authority
  • a housing team
  • a probation service
  • an addiction unit
  • a health authority.

Where will the support be delivered?

The support provided can range from 1 up to 10 hrs based on individual needs. These sessions can be conducted in the service users home or another appropriate place which can be decided by the support worker or the service user.

 

How long will the support last?

Our young people service is funded to provide support to individuals for up to two years though this figure is not definitive, and may be extended depending on a residents need s and motivation.

 

Support

A support plan will be developed within the first week, between the key worker and resident. The support will include advice and assistance with planned move-on and resettlement.

 

How we will work with you

We will provide a named key worker to help you to develop a support plan to identify your  goals and how to work towards them. These will be reviewed regularly to ensure that the right support is being offered.

 

What support will be offered?

  • Help with accessing alternative support and or services.
  • Help to find/ access community resources in your area.
  • Assistance with claiming the right benefits and filling in forms.
  • Help with bills, dealing with debt and maximising your money.
  • Advice on tenancy issues and managing your home.
  • Support to find education, training employment and voluntary work.
  • Advice and information on health.
  • General life skills.
  • Support and assistance to maintain tenancy.
