Aims of the service

To provide a service to those people who still require a degree of support to enable them to live independently within the community.

 

Who can apply for the service?

Anyone who:

  • is aged  between 16 and 65
  • is a single person with support needs
  • is eligible for state benefits
  • presents no significant risk of harming self or others
  • is able to make an informed choice.

 

How to access the service

Access to the service can be via:

  • self referral
  • local authority
  • local housing team
  • probation service
  • addiction units
  • local health authority.

What is the accommodation like?

Wilkins Drive is a purpose-built semi independent unit  of 12 fully furnished flat lets with cooking and washing up facilities for single homeless people.
There is shared use of bathrooms, toilets, showers, laundry and communal lounge.
The service is provided 9-5 five days a week and there is also an on-call service available out of hours.

 

How long will the support last?

Wilkins Drive is funded to provide support to residents for up to 2 years though this figures is not definitive, and may be extended depending on a residents need s and motivation.

 

Support

A support plan will be developed within the first week, between the key worker and resident. The support will include advice and assistance with planned move-on and resettlement.

 

How we will work with you

A named key worker to help you to develop a support plan to identify your  goals and how to work towards them. These will be reviewed regularly to ensure  that the right support is being offered.


What support will be offered?

  • Help with accessing mental health support and services.
  • Help to find community resources and other support services in your area.
  • Assistance with claiming the right benefits and filling in forms.
  • Help with bills, dealing with debt and maximising your money.
  • Advice on tenancy issues and managing your home.
  • Support to find education, training employment and voluntary work.
  • Advice and information on health.
  • General life skills.
  • Assistance with resettlement and move  on options when you are ready to move on.
