Who can be referred to the service?

Anyone aged 18–65 who is currently living in Southwark with mental health and substance misuse problems.

The focus of all support within the project is to improve independent living skills and to promote greater independence in the community.

 

How to access the service?

Your referral must be agreed by the SLaM funding panel, and a MST Support Application Form needs to be completed. You need to be on a CPA and willing to engage with the supported accommodation services that we offer.

 

What is the accommodation like?

Southwark Bridge Road is a 24 hour staffed project, situated in the London Borough of Southwark, 5 min walk from Borough Tube Station. The Project consists of two housing schemes; medium support, and self-contained bedsits. The building is organised into five, five-bedroom flats, and three self contained bedsits, a total of 28 bed spaces.

Herons Court is a step down service, consisting of 12 one-bedroom flats, situated on Pomeroy Street and 6 one-bedroom flats at Meeting House Lane. The Project is staffed weekdays between 9am—7pm and provides low support.

 

Where will the support be delivered?

You will be allocated a Key Worker who will work with you directly. The frequency of your meetings will be based to according to your scheme’s requirement.

These meetings are to discuss your support needs and how they will be met. Key work sessions can be held in the office or in the community (i.e. Cafés, library etc…) depending on when you would like to meet. 

 

How long will the support last?

The support that we provide will normally last for up to two years at Southwark Bridge Road, at Herons Court this can be up to 5 years. Residents will be supported to be on the Southwark Housing Register within two to three months from the moving in date. When you feel confident about living more independently we will work with you to move on to more appropriate accommodation. The service is funded by Supporting People Team at Southwark Council.

 

How we will work with you

Together we will identify what help you need and want, how we can best support you and how the support will be provided. The outcome based support planning process is where we agree what support you need help with, and who will help you to achieve your goals. The support plan will be reviewed six monthly / when your circumstances change.

  • Emotional support, counselling and advice
  • Help in gaining access to other services
  • Help in establishing social contacts and activities
  • Help in establishing personal safety and security
  • Supervision of physical / mental health and well being
  • Supervising and monitoring medication- with goal of self medication
  • Peer support and befriending
  • Help in finding other accommodation
  • Risk assessment and Risk management plan
  • Individual Support Plan
