Promoting independence at home for older people

What we do
Our aim is to help you live independently in the community.  We will support you with housing related issues to enable you to live safely and comfortably in your own home. The service is:

  • free
  • confidential
  • flexible
  • provided at your home

Who is the service for?
Anyone aged 60 and above living within the Nottingham City area and needing support with housing related issues.

What support will be offered?
Some of the things we can help with include:

  • maintaining and/or finding suitable accommodation
  • help with claiming benefits and filling in forms
  • resolving pension problems
  • advice on budgeting
  • getting aids and adaptations fitted
  • advice on home safety and security
  • contacting other services to arrange repairs to your home
  • coping with poor health and/or disability
  • dealing with loss or bereavement
  • coping with the effects of crime
  • general life skills and confidence building
  • finding social clubs or ways of meeting people
  • finding other services that meet your needs and cultural requirements

How we will work with you
A support worker will be allocated to you who will visit you at home or a suitable community setting. He/she will agree a support plan with you which will list all your support needs and plans for achieving your goals. This plan will be regularly reviewed to discuss your progress. If we need to work in partnership with other agencies to help you, we will seek your permission before contacting them.

How long will the support last?
While there is no definite time limit to how long you can receive support, the service is designed to be short term. When your support needs have been met, we will agree a date with you to end the service. You can contact us if your circumstances change in the future and you need support again.

How to access the service
You can:

  • contact us yourself
  • get a family member, friend or neighbour to contact us
  • ask your GP or nurse to make a referral
  • ask your carer, social worker, warden or any other person who supports you to make a referral
