Rushcliffe floating support service

Aims of the service

The aim of the service is to work with you to resolve any current and ongoing issues that are preventing you from successfully maintaining your tenancy.

The purpose is to promote your independence by giving you the skills to deal confidently with these problems. We hope that you will then have the ability to tackle any future difficulties independently.

 

Who can access this service?

Individuals who live in the Rushcliffe area. Please see “Who can apply for the service?”

 

Where will the support be delivered?

Support is generally provided in your home although you may agree to meet at our offices or other location in your local community.

 

How long will the support last?

The support can last for up to 2 years but often goals are achieved before this time, enabling people to live more independently.

 

How we will work with you

You and your support worker will identify the areas you would like to work on by completing a support plan. We  will do this by agreeing the goals you would like to achieve and the actions which will help you to do this. The support plan will be regularly reviewed to discuss your progress and plan any future aims.

 

What support will be offered?

Help with claiming benefits, and filling in forms.

  • Advice on budgeting/debts and rent/council tax arrears.
  • Support to identify education, training, employment and voluntary work.
  • Help to contact other agencies.
  • Undertake signposting and referrals to other appropriate agencies, services and community resources.
  • Support with move on, setting up and maintaining tenancies.
  • Support to access health advice.
  • Help to settle disputes if you are having difficulties with neighbours.
  • Any other support that will enable you to keep your tenancy.

Who can apply for the service?

People with:

  • mental health needs
  • alcohol problems
  • physical disabilities
  • older persons
  • other vulnerable groups.

 

How to access the service

If you feel you meet the criteria, you can be referred through various agencies or you can make a self-referral. Please contact us on 0115 9887396 for our referral forms or any questions you may have.

 

How is the support service funded?

The service is funded by Supporting People. You do not have to pay anything for this service.
