Aims of the service

  • To provide ongoing personal care and assistance for the individual service users physical and emotional needs.
  • To provide support to individuals to carry out their day to day activities and access community facilities if desired.

 

Who can apply for the service?

Anyone over 18 with a physical disability and registered with Chesterfield Borough Council housing department.

Anyone who requires support to live in the community.

 

How to access the service

Individuals can contact the project directly or a referral can be made via the local Social Services Department. A care plan and risk assessment will need to be provided.

Individuals aged over 18 with a physical disability and on the local authority housing waiting list.

 

What is the accommodation like?

The accommodation is a single storey building with five self contained flats for people with a physical disability and is owned by Sutton Affinity.

The flats consist of one bedroom, bathroom, kitchen and lounge. The flats benefit from a call system direct to the carer’s bedroom and is also linked to Chesterfield Careline.

 

How long will the support last?

The support will last for as long as you are able to continue living at Paisley Close.

 

How we will work with you

Once your referral has been approved a full assessment and up to date care plan must be submitted by the referrer (social worker, care manager etc.)  From these documents we would develop a support plan and risk assessment.

Support staff work on a shift system and staffing levels will be specifically geared to meet the needs of the individual.

What support will be offered?

We are able to provide the following services:

  • assistance with eating/drinking
  • encourage independent living skills
  • assistance with menu/meal preparation
  • household shopping/budget management
  • assistance with administration of prescribed medication
  • visits to doctor/hospital
  • cover for day service closures
  • personal care
  • sleep-in cover.
