Aims of the service

This service provides support to vulnerable adults with a history of  enduring mental health to enable them to live independently in the community and make sure their tenancies are sustained. It supports people to develop the skills necessary to move onto independent accommodation.

 

Who can apply for the service?

Anyone aged 18 years  or over who have a mental health diagnosis under an enhanced CPA and also in need of housing related support to maintain their  their tenancies.

 

How to access the service

You must be referred by the Lambeth Mental Health Placement  Panel who works  in conjunction with the SNAP team.

 

What is the accommodation like and where will the support be delivered?

Northlands Street is a block of 3 self contained flats. You will receive support in your home by regular arranged support visits. You will be allocated a support worker who will work with you directly. They will meet with you at least once a week. Duty staff at MHT House office are available to assist you Monday to Friday between 9am and 5pm.

 

How long will the support last?

This service is being developed to provide long term supported housing to people with an enduring mental health need. Support will be provided to service users while they continue to demonstrate a housing support need.

The service is funded by the Supporting People Team at Lambeth Council. You should not have to pay for the service that we provide

 

How we will work with you

Together we will identify what help you need, how we can best support you and how the support will be delivered. This will be done using our outcome based support planning process, which discusses the area where you need help, and who will help you to achieve your goal/s. The support plan will also be reviewed on a regular basis with you. This may be in addition to any existing support you may receive from other agencies.

 

What support will be offered?

  • Help to access specialist support and services.
  • Help to access community resources and other support services in your area.
  • Assistance to claim the correct benefits and help completing forms.
  • Help with bills, clearing debt and maximising money.
  • Advice on tenancy issues and managing your home.
  • Support to find education, training, employment and voluntary work.
  • Advice and information on health.
  • General life skills.
  • Assistance with resettlement and move on options when you are ready.
