Floating Support (Mental Health)

Aims of the service

To provide support to enable individuals with a diagnosed mental health difficulty to maintain their tenancy.

 

Who can apply for the service?

Anyone who:

  • has a recognised Mental Health diagnosis
  • is aged between 18 and 65
  • has an enhanced level of Care Programme Approach (CPA)
  • is eligible for state benefits
  • who requires social rather than health based care.

Applicants must also have a current risk assessment and management plan and been offered or is living in their own accommodation.

 

How to access the service

We are only able to accept referrals from a health or social care professional including those in probation, housing and the substance misuse fields. Their contact details can be provided on request. 

We regret that we are unable to accept self referrals.

 

Where will the support be delivered?

The support provided  can range from 1 up to10 hrs based on individual needs. These session can conducted in the service users home or another appropriate place which can be decided by the support worker.

 

How long will the support last?

Our mental health floating support is funded to provide support to residents for up to 2 years though this figures is not definitive, and may be extended depending on a residents need and motivation.

 

Support

A support plan will be developed within the first week, of acceptance on to the service between the key worker and resident. The support will include advice and assistance to maintain tenancy.

 

How we will work with you

All staff are committed to working in an anti oppressive manner and work towards combating the prejudice and stigma surrounding mental illness. Staff will be available for 7.25 hrs a day Monday to Friday. The service is designed to be flexible to your needs and if you prefer to be visited in the evenings or at weekends this can be arranged. An on call service is available evenings and weekends.

  • Help with and support with maintaining involvement with mental health support and services.
  • Help to find community resources and other support services in your area.
  • Assistance with claiming the right benefits and filling in forms.
  • Help with bills, dealing with debt and maximising your money.
  • Advice on tenancy issues and managing your home.
  • Support to find education, training employment and voluntary work.
  • Advice and information on health.
  • General life skills.
  • Assistance with and support to successfully maintain tenancy.
