Aims of the service

We offer a service for Southwark residents aged 18–65 who experience severe and enduring mental health needs.

 

How can I access the service?

You can access the service using the Community Opportunities Referral Form used by social workers and community psychiatric nurses at Community Mental Health Teams.

 

How the service operates

We are committed to:

  • involving members as much as we can in the service
  • agreeing individual support plans
  • taking part in meetings
  • suggesting and helping to run group activities
  • becoming involved in the wider community.

Staff work in an individual, person-centred way to plan support, helping members to identify their goals and work towards a more healthy, independent and fulfilling life.

 

Activities

We offer a range of activities at the day centre such as:

  • a photography group – run by members for members
  • a women's group
  • outings to places of interest and visits to community resources such as libraries and volunteering agencies using public transport
  • a rolling programme of creative groups to enhance and teach new skills, including a letters and forms group and a book club.

 

Who we work with

We work closely with Community Mental Health Teams and other agencies delivering packages of support and attend CPA meetings when possible.

We support members’ involvement in community opportunities, including Capital Volunteering, Southwark College, South Bank University, CoolTan Arts, Time & Talents and Pecan.

 

Confidentiality

We keep all information about you confidential within the staff team, only sharing it with other professionals involved in your care.
