Include: Learning disability floating support in Nottingham City

The aim of this service is to provide a floating support service to adults who have a mild to moderate learning disability (including autism/Asperger’s syndrome).

 

Where and when will the support be delivered?

The length and frequency of all support visits will be discussed and agreed with you in advance. Support visits usually take place in your own home but can also take place in other settings in the community.

Support is usually delivered during office hours but support outside of these times is available if required.

An emergency on call service is available for support and advice during the evenings and at weekends throughout the year.

 

What is floating support ?

The Include Floating Support service is a short term service. Floating supports primary aim is to prevent homelessness and to assist you to find and maintain your tenancy.

We can support you for up to two years but not everyone will require support for this long.

Staff work with a wide variety of other agencies to ensure that appropriate support is in place.

The service is free for people who meet our criteria.

 

How we will work with you

We will provide a low to medium level floating support service to people who live in Nottingham City.

You can access between half an hour and ten hours support each week depending upon your needs.

You will have a named keyworker. Your support hours can increase and decrease as your needs change.

A support plan will be drawn up between the support worker and you, this will be reviewed regularly with you.

 

What support will be offered?

We will support you to:

  • apply for housing and set up home
  • maintain your existing tenancy, preventing and dealing with any rent arrears
  • access welfare benefits, assist in completing forms and maximise your income
  • develop your life skills
  • help with personal budgeting and household management
  • identify education, training and employment opportunities
  • improve your confidence including emotional support
  • enable you to access leisure opportunities and become more involved in your community
  • access relevant health services
  • deal with problems with neighbours.

Who can apply for the service?

Applicants should have a mild to moderate learning disability, autism or Asperger’s syndrome.

Applicants who require support with housing related issues, resettlement or tenancy sustainment.

Applicants should be aged over 16 or over and living in the Nottingham city area.

Applicants should be willing to accept regular support from support workers.

 

How to access the service

Referral forms can be obtained by contacting the Include team at the address overleaf.

Self-referrals will be accepted, or you can be referred via other agencies including social workers, housing officers.

We may ask to speak with others who provide you with support in order to understand your needs.
