Aims of the service

  • To provide a home care service for four people with learning difficulties.
  • To provide support to individuals carrying out their day to day activities in order to promote emotional, physical and mental based on individual care plans.

 

Who can apply for the service?

  • Individuals who have a learning disability.
  • Individuals who require support to live in the community.

How to access the service

Individuals can contact the project directly or a referral can be via the local Social Services Department. A care plan and risk assessment will need to be provided.

 

What is the accommodation like?

The accommodation is a purpose built bungalow and has four bedrooms, one bathroom one shower room and an open plan lounge, kitchen, and conservatory area. There is also a staff room/bedroom. The bungalow has a level access shower, closomat toilet and powered bath seat.

Service users have their own tenancy agreement for rental of their room and use of communal facilities. Rent is payable on a weekly basis to Metropolitan Support Trust.

 

How long will the support last?

The support will last for as long as you are able to continue living at Hassop Close.

 

How we will work with you

Once your referral has been approved a full assessment and up to date care plan must be submitted by the referrer (social worker, care manager etc.)  From the Social Services care plan we would develop a support plan with you which would identify the best way to support you.

Support staff work on a shift system and staffing levels will be specifically meet the needs of the individual.

 

What support will be offered?

The Hassop Close scheme has been designed to meet the needs of four users with learning difficulties. 24 hour care is available and the following services are provided.

  • Assistance with eating/drinking.
  • Encourage independent living skills.
  • Assistance with menu and meal preparation.
  • Household shopping/budget management.
  • Storage and assistance with administration of prescribed medication.
  • Visits to doctors/hospital.
  • Personal Care.
  • Social activities.
  • Cover for day service closures.
