Aims of the service

To provide a service to those people who  require a degree of support to enable them to regain a state of self reliance and independence in the community.

 

Who can apply for the service?

Anyone who:

  • is aged  between 16 and 65
  • finds themselves in a position of being homeless
  • is male or  female
  • is eligible for state benefits.

How to access the service

Access to the service can be via:

  • self-referral
  • local authority
  • local housing team
  • the probation service
  • addiction units
  • local health authority.

What is the accommodation like?

Hartington House is a 24hr, 32-unit hostel which provides direct access to people who find themselves homeless. The hostel is situated within walking distance of all local amenities and the town centre.

Each unit is fully furnished with the shared use of kitchen, bathrooms, toilets, showers, laundry communal lounge and dinning room. The project also provides meals.

 

How long will the support last?

Hartington House is funded to provide support to residents for up to 2 years though this figure is not definitive, and may be extended depending on a resident’s needs and motivation. The average length of stay is usually between 3-12 months.

 

Support

A support plan will be developed within the first week, between the key worker and resident. The support will include advice and assistance with planned move-on and resettlement.

 

How we will work with you

A named key worker to help you to develop a support plan to identify your goals and how to work towards them. These will be reviewed regularly to ensure that the right support is being offered.

 

What support will be offered?

  • Help with accessing mental health support and services.
  • Help to find community resources and other support services in your area.
  • Assistance with claiming the right benefits and filling in forms.
  • Help with bills, dealing with debt and maximising your money.
  • Advice on tenancy issues and managing your home.
  • Support to find education, training employment and voluntary work.
  • Advice and information on health.
  • General life skills.
  • Assistance with resettlement and move on options when you are ready to move on.
