Gedling floating support service

Aims of the service

To provide a floating support service to adults who have a mild learning disability or other long term vulnerability.

 

Who can eligible to apply for the service?

Anyone can apply for the service who requires support with housing related issues and who has a mild learning disability or other long term vulnerability. Applicants should be aged over 18 and must be living in Gedling borough, they may be living in any tenure. Applicants should be willing to accept and engage with regular support from support workers. We accept referral from couples, and single people and those with children.

 

How to access the service

Referral forms can be obtained by contacting the supported tenancies team at the address overleaf. Self referrals and agency referrals will be accepted. We will arrange to meet with you to talk in more detail about your support needs. We may ask to speak with others who provide you with support in order to understand your needs.

 

Where and when will the support be delivered?

The length and frequency of all support visits will be discussed and agreed with you in advance. Support visits and times are flexible. Support visits usually take place in your own home but can also take place in other settings in the community. Support is usually delivered during office hours but support outside of these times is available if required. An emergency on call service is available for support and advice during the evenings and at weekends.

 

How we will work with you

We will provide a low to medium level floating support service to people who live in Gedling Borough. Between four and ten hours support can be offered each week depending upon individual needs. Support hours can increase and decrease as needs change. All service users will have named key worker. A support plan will be drawn up between the support worker and service user, this will be reviewed every three months.

 

What kind of support will be offered?

We will support you to:

  • apply for housing and set up home
  • maintain your existing tenancy, preventing and dealing with any rent arrears
  • access welfare benefits, assist in completing forms and maximise your income
  • develop your life skills
  • help with personal budgeting and household management
  • identify education, training and employment opportunities
  • improve your confidence including emotional support
  • enable you to access leisure opportunities and become more involved in your community
  • access relevant health services
  • deal with problems with neighbours.

 

How long will the support last?

The duration of the service provided, will be always be based upon individual service users needs.

 

How are support places allocated?

Once assessed and accepted for a support place. Applicants are placed on a waiting list. Priority will be given to applications coming from Gedling ACMT. Referrals from CLDTs will be accepted in circumstances where it is considered that the person has mild learning disabilities but have been accepted by a CLDT due to other complicating factors.
