Who can apply for the service?

Anyone aged 18-65 years who is currently living in the London Borough of Southwark with a mental health diagnosis and receiving case management from the High Support Rehab Team.

 

How to access the service

You must be referred by Southwark High Support Rehabilitation Team. You should contact your Care Co-ordinator to check if you meet their criteria.

 

Where is the support delivered?

De Crespigny Park comprises of twelve self contained one bedroom flats. In addition it has a shared garden and some communal areas. The project is located in Southwark, close to local transport and shops. You will be allocated a Keyworker who will work with you very closely and meet with you for a support session at least once a week. Support staff will be on site at all times and available to meet with you if you need any advice or guidance.

 

How we will work with you

We will work with you to identify your goals for the future. Together we will create a Support Plan which will detail your goals and support your needs to achieve them.

The support plan will cover the following areas.

  • Emotional support/motivation.
  • Medication.
  • Budgeting and life skills.
  • Benefits help and advice.
  • Access to work and education opportunities.
  • Social and leisure activities.
  • Move-on and resettlement to other accommodation.
  • Service user involvement.
  • Health and safety.
  • Maintenance.

 

What other support will be offered?

As well as Support Workers, DCP employs two Support Time and Recovery Workers, who can help with in-house literacy training and basic IT skills and help service users get involved in the community. Other therapeutic activities such as days out, trips and barbecues are regularly organised. Recently there has been a day trip to France and a holiday to Spain. At DCP Service Users are encouraged to be part of their own recovery. Using the Recovery Model we aim to help people with mental health problems to move beyond mere survival and existence, to being able to carry out activities and develop relationships that give their lives meaning. Recovery emphasises that while people may not have full control over their symptoms, they can have full control over their lives.

 

How long will the support last?

The support we provide will normally last for up to  2 years. When you feel confident about managing for yourself, we will work with you to find more appropriate housing for you to move on to.

 

How is the support service funded?

The service is funded by the South London and Maudsley NHS Trust, and the Southwark Supporting People Team. You should not have to pay for the support we provide.
