Aims of the service

The Learning Disability service based at Briarwood court provides housing with support for 11 people with a learning disability.

Briarwood Court aims to enable people to live as independently as possible in their own home. Whilst supporting them to take control of their own lives.

 

Who can apply for the service?

Service users whose learning disability is their primary support need. The service only accepts applications from singles, we are unable to accept referrals from people who have children living them. There is no particular time limit for the length of tenancy agreement in either the flats or shared house.

 

How to access the service

All referrals should be made via social services. If you do not currently have a social worker you should contact your local social services office who should be able to help.

 

What is the accommodation like?

Briarwood consists of seven self contained single flats and a 4 bedded shared house.
We provide 24hr staff cover on a shift basis and emergency on call facilities. White goods are provided within the flats and the shared house, there is also a communal washing machine and tumble drier. Briarwood Court is situated in Sherwood within easy access to local shops and services.

 

How long will the service last?

Support continues for as long as it meets an individuals needs. It is hoped that tenants will develop the necessary skills and confidence required to move onto independent living within the community.

 

Where will the support be delivered?

Support will be carried out within the tenants flat or house unless an individual requests otherwise. Tenants are allocated a key worker who will work with them to identify their needs and how they will best be met within the service.

 

How we will work with you

We will provide 15hours 1:1 support each week, support needs will be identified and discussed with the tenant. All support will be offered in a person centred way and will be reviewed on a regular basis.

 

What support will be offered?

Support is tailored to individual needs, tenants can access support in the following areas:

  • support to maintain tenancy
  • support to improve life skills including cooking, shopping and cleaning
  • help with budgeting and paying bills
  • support with claiming benefits, filling in forms and maximising  income
  • support to identify education, training and employment opportunities
  • support to build on self confidence including emotional support
  • support to make and attend health care appointments
  • support to access leisure & social opportunities.
