Aims of the service

  • To provide a 24hr facility for individuals with a learning disability.
  • To enable and assist them to live independently by providing appropriate opportunities for the development of their spiritual, physical, emotional, and intellectual needs.

Who can apply for the service?

Any one who:

  • has been diagnosed with a learning disability
  • is between 16 and 65
  • requires 24-hour support
  • has an up to date risk and personal assessment completed by a health  or social  care worker.

We are only able to accept referrals from a health or social services representative. Their contact details are:

We regret that we are unable to accept self referrals or referrals from any other agencies.

 

What is the accommodation like?

Ashbourne Road is an Edwardian style 5 bedroom family house, situated in the centre of Derby, within  walking distance of the city centre and all local amenities.

How long will the support last?

Ashbourne Road is a long term service and the level and duration of the service provided is based on individual support plans.
Should there be a need for someone to move on this will be done within a multi disciplinary format with involvement from the service user, their representative/advocate, their key worker and a representative from the statutory agency.
Staff at Ashbourne Road work with a range of agencies to ensure service users access the most appropriate services available to them.

How we will work with you

Through the development of an individual  support plan, the key worker will help the service user to work towards achieving identified goals.
These will be reviewed regularly to ensure the right level of support is being offered.

What support will be offered?

The support from staff at Ashbourne Road includes:

  • help with accessing mental and physical health support  services
  • help to find community resources and other support services in your area
  • assistance with claiming the right benefits and filling in forms
  • help with bills, dealing with debt and maximising your money
  • advice on tenancy issues and managing your home
  • support to find education, training employment and voluntary work
  • advice and information on health
  • general life skills
  • support and assistance to move on to more appropriate accommodation as and when necessary.
