This service provides support to vulnerable adults with a history of homelessness or rough sleepers to enable them to live independently in the community. It supports people to develop the skills necessary to move onto independent accommodation.
Anyone aged 18–65 years who is currently a rough sleeper and has a recent history of homelessness.
You must be referred by the Street Population Outreach Team (SPOT) or a specialist homelessness agency and have been assigned a chain number and be linked to the clearing house.
Acre Lane is a block of 7 studio flats with an on site office. You will receive support in your home by regular arranged support visits. You will be allocated a support worker who will work with you directly. They will meet with you at least once a week. Other staff at the office are available to assist you Monday to Friday between 9am and 5pm
The support that we provide will normally last for up to 1 year. When you feel confident about living more independently we will work with you, alongside the Clearing House to find appropriate housing for you to move on to. The service is funded by the Supporting People Team at Lambeth Council. You should not have to pay for the service that we provide.
Together we will identify what help you need, how we can best support you and how the support will be delivered. This will be done using our outcome based support planning process, which discusses the area where you need help, and who will help you to achieve your goal/s. The support plan will also be reviewed on a regular basis with you. This may be in addition to any existing support you may receive from other agencies.
